Job Summary

The Acute Care Nurse Coordinator for the Advanced Heart Failure Comprehensive Care Center (AHF CCC) will function in numerous roles: 1) The nurse coordinator will work closely with the AHF ambulatory Pre- Heart Transplant coordinators to expedite inpatient heart transplant evaluations for acute and critically ill patients. He/she/they may also provide direct care to hospitalized patients with Advanced Heart Failure, Heart Transplant, Pulmonary Hypertension and Mechanical Circulatory Support. 2) The Nurse Coordinator will provide nursing supervision, monitoring, and management of patients with mechanical circulatory support devices while undergoing procedures requiring anesthesia. Transporting and traveling throughout the hospital with acutely ill patients is a requirement of this role. 3) The Nurse Coordinator will also support the Post Heart Transplant patients by teaching the patient and family about Post Heart Transplant care. A key role will be supporting and optimizing the transitions of care to home with close attention to assessing readmission risks.

Key components of the role include education regarding disease process, medications, lifestyle management, and self-monitoring and safety awareness. The Nurse Coordinator participates in daily rounds with the inpatient Advanced Heart Failure Service, as well as Multidisciplinary rounds for CTS, MCS and AHF. The Nurse Coordinator will work very closely with the outpatient AHFCCC teams and will participate in daily huddles and communicate a plan of care in a clear hand-off template to the outpatient AHF CCC clinical team to promote a safe and smooth transition from the hospital to home, ensure all aspects of care following discharge have been reviewed and optimized to prevent decompensation and/or readmission.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $91.20 - $118.34 (Hourly Rate).

Required Qualifications

  • Five years of Cardiac ICU nursing employment experience including care of Advanced Heart Failure patients, heart transplant, and those with MCS devices. with evidence of sound knowledge of hemodynamics
  • Working knowledge and independent management of all temporary and durable Mechanical Circulatory Support Devices
  • Ability to assist with transport of patients on acute and durable MCS devices and the physical ability to wear a lead apron and other lead protective devices as needed when present in procedures with radiation exposure risk.
  • History of experience with nursing education and demonstrated exemplary patient teaching skills and transferring of critical knowledge to patient and families with closed loop communication. Knowledge of Teach Back Method
  • Excellent organizational skills, proficiency in meeting deadlines, ability to work independently, prioritize workload and manage numerous concurrent tasks in high stress environment
  • Demonstrated excellent written and verbal communication skills
  • The flexibility to orient and work at all UCSF Medical Center locations in person on campus


Preferred Qualifications

  • Bachelor's degree in Nursing
  • One to three years of outpatient ambulatory cardiology/heart failure/transplant clinic experience
  • History of Operating Room experience with acute and durable MCS devices (LVAD, Impella, ECMO) implantation support
  • Working knowledge in assessing advanced heart failure/cardiac disease
  • ECMO/ECLS Certification, Certification in Heart Failure Nursing, or Certification in Transplant Nursing


License/Certification

  • Active Registered Nurse licensure in the State of California
  • Active American Heart Association BLS certification
  • Active American Heart Association ACLS certification
