Job Title: Licensed Practical Nurse (LPN) - Full Time
Job Specialty: Long-Term Care (LTC)
Job Duration: 12 Months
Shift: Full-Time Day/Night Rotation (7:00 AM - 3:30 PM / 3:00 PM - 11:30 PM), 5 days a week, including every other weekend and holidays
Guaranteed Hours: Not specified
Experience: Minimum 1 year of experience as an LPN in a long-term care facility
License: New York LPN Licensure
Certifications: Basic Life Support (BLS) certified through AHA or RQI
Must-Have: Ability to provide care to adult and geriatric patient groups
Job Description:
- Demonstrate knowledge and skills necessary to provide age-appropriate care for patients in the assigned unit.
- Assess data reflective of the patient's status to interpret information needed for patient care specific to age-related needs.
- Provide direct and indirect skilled nursing care under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with nursing staff, physicians, and ancillary staff.
- Participate in continuing education and in-service programs to maintain knowledge and clinical expertise.
- Collect data on residents' physical and psychosocial conditions and communicate findings to the RN and Interdisciplinary Team.
- Contribute to the creation of a culturally relevant and developmentally appropriate care plan.
- Recommend revisions to care plans based on evaluation of resident outcomes and coordinate with the Interdisciplinary Team.
- Ensure customer satisfaction by collaborating with residents, families, and the healthcare team.
- Mentor and orient new staff while considering safety, cost, and impact in care planning.
- Lead under the direction of the RN in the professional practice setting.
- Anticipate occupational exposure to bloodborne pathogens with relevant safety precautions.
