Job Description
PURPOSE OF POSITION:
This class encompasses professional staff with Supervisory, clinical or administrative work within a pharmacy department, with varying degrees of latitude for independent initiative and judgment. Health System Pharmacists adhere to Federal, State and local laws and other regulatory requirements regarding Pharmacy and institutional practice.
DUTIES & RESPONSIBILITIES:
1. Fills prescriptions and patient orders, resolves discrepancies in patient therapy, and follows-up on problems.
2. Utilizes clinical data in evaluating physician orders to ensure appropriate therapy.
3. Provides and documents drug information and patient counseling.
4. Documents interventions, adverse drug reactions (ADRs) and medication errors.
5. Reviews clinical information to assess appropriateness of patient therapy.
6. Resolves discrepancies in patient therapy and following up on problems in a timely manner.
7. Provides appropriate drug information to hospital staff.
8. Conducts effective patient drug counseling
9. Records and reviews patient orders via the computer system.
10. Prepares and maintains files and reports.
11. Participates in Pharmacy staff in-services and hospital wide meetings.
12. Communicates effectively and provide relevant information to colleagues and other health care workers regarding patient therapy or handoff information.
13. Ability to collect data and prepare reports.
14. Knowledge of the Pharmacy computer systems and other related computerized systems
Minimum Qualifications
Possession of a valid NY State license to practice pharmacy.
