POSITION SUMMARY
The Lead/Coordinator Advanced Practice Provider (APP) answers the Division Chief of Vascular, Endovascular and Wound Surgery to provide primary and acute medical care in a specialized area specific to vascular surgery and wound management. The Lead Advanced Practice Provider is responsible for assessment, diagnosis, and treatment plan development of high-risk patients. Helps to create management protocols for patients under the direction of the Chief Division of Vascular/ Wound Surgery. The Lead APP/Coordinator facilitates APP recruitment, onboarding, schedule oversight, and role evaluation of all APPs within the division and follows on productivity of all APPs within the division. The Lead APP/Coordinator provides coordination and integration with the UTMC wound services and Hospital Quality office and provides leadership and guidance in daily operational workflows, staffing, program development, revenue cycle, outreach, education, and marketing.

ESSENTIAL JOB FUNCTIONS AND ACCOUNTABILITIES
- Assumes responsibility for patient management care in the fields of vascular surgery and wound management under collaboration with physicians according to a predetermined standard care arrangement and current standards of clinical care.
- Obtains a comprehensive and systematic health history, including physical, social, and emotional components.
- Performs full physical examination or focused evaluation based on presenting problem.
- Initiates and interprets appropriate diagnostic testing.
- Requests physician consultation when a problem calls for knowledge or skill outside of his/her authorized level of care.
- Utilizes protocols mutually agreed upon with collaborating physicians.
- Develops, implements, and evaluates individualized plans of care based on nursing history, physical assessment, and patient consultation.
- Prescribes medications as appropriate though Ohio Law.
- Arranges for appropriate follow-up.
- Coordinates care for patient across care settings
- Assists with clinical procedures, patient assessment, management plans and protocols with the help of the vascular surgeons, utilizing current medical and nursing resources.
- Evaluates needs of individuals for community and/or University referral resources and makes appropriate referrals.
- Collaborates with other health professionals in planning and ensuring continuity of care.
- Assists in identifying and planning for needed health promotion and disease prevention programs including screening and community teaching. Helps increase the outreach of the vascular surgery and wound service of the Division of Vascular surgery.
- Acts as a resource person to provide health information and/or educational presentations
- Complies with institutional, federal, state, and nursing facility or LTACH regulatory standards in the delivery of patient care.
- Maintains knowledge and skills at a current, optimum level and demonstrates a commitment to professional development by participating in professional activities, educational programs and continuing education through independent reading and attending other learning programs.
- Participates in performance improvement projects.
REQUIRED QUALIFICATIONS
Education:
- Graduate of an accredited Nurse Practitioner Program or Physician Assistant Program
License and Certification:
- State of Ohio Licensure-Nurse Practitioner or Physician Assistant
- State of Ohio Licensure-Registered Nurse
- National Certification as a Nurse Practitioner or Physician Assistant
- BLS Certified
- Eligible to obtain and/or maintain Certificate to Prescribe/DEA
Years of Experience:
- A minimum of 3 years working as an Nurse Practitioner or Physician Assistant
PREFERRED QUALIFICATIONS
- 10 years of Advanced Practice experience with experience in vascular and wound surgery.
- 5 years of clinical and administrative leadership experience.
WORKING CONDITIONS
- Primarily inside in well-lighted, well-ventilated areas. High stress environment where exposure to bloodborne pathogens and other infectious agents, unpleasant odors, limited space, dust and fumes, hazardous chemicals, radiation risk and loud noise may exist.
