Job Description
Job Title: Registered Nurse - Hospice
Job Specialty: Hospice Care
Job Duration: 13 weeks
Shift: Monday to Friday, 8:00 AM - 5:00 PM
Guaranteed Hours: 40 hours per week
Experience: A minimum of one year as an RN in acute care hospital, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered in individual cases.
License: Current license to practice nursing in the state of employment
Certifications: BLS (American Heart Association), Current automobile insurance, and valid driver's license
Must-Have:
- Experience with terminally ill patients and their families
- Knowledge of hospice care and principles
- Excellent patient assessment skills
- Knowledge of community resources
- Good oral and written communication skills
- Documentation and program management skills
- Ability to effectively operate nursing assessment tools, computers, facsimile equipment, copier, and cell phone/beeper
- Transportation for work purposes
Job Description:
- Provide direct patient care services to hospice patients and supportive care to their families.
- Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
- Assure the highest quality of hospice care as documented in accordance with the Plan of Care.
- Perform initial and ongoing assessments of patients.
- Develop and implement the Hospice Plan of Care in collaboration with the physician and Interdisciplinary Group.
- Work closely with the Interdisciplinary Group to provide comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
- Report any changes in patient condition to both the Attending Physician and Hospice Physician.
- Maintain accurate medical records, including interdisciplinary notes, Plans of Care, and assessments.
- Practice infection control measures and standard precautions.
- Monitor scheduling of all staff members.
- Participate in the Quality Assessment and Performance Improvement Program.
- May participate in the agency's on-call schedule.
- Promote the company's core values consistently.
- Complete required compliance and annual training.
- Attend staff meetings and maintain expertise in managing pain and symptoms related to the dying process.
- Make routine supervisory visits and perform ongoing reviews of assigned patient clinical records.
- Maintain contact and communication with patients' families to provide information, support, and continuity of care.
- Educate patients, families, or caregivers regarding the care needed as per the Plan of Care.
- Supervise Aides and make home visits at least every two weeks to assess the care and services provided.
- Prepare written assignments for Aides.
