Job Description
Job Title: LCSW - Discharge Planner/Case Manager
Job Specialty: Licensed Clinical Social Worker
Job Duration: 13 weeks
Shift: Day, 5x8-Hour (08:00 - 16:30)
Guaranteed Hours: [Not specified]
Experience: Minimum of 2 years in discharge planning, including admission criteria review, benefits eligibility, care coordination, conducting needs assessments, and order DME. Experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, and acute hospital experience. Prior charge experience preferred. Completion of at least one travel assignment.
License: Must have LCSW, LMSW, or LGSW licensure. Certain positions may accept pending licensure.
Certifications: BLS required
Must-Have: Familiarity with CMS, HIPAA, JCC; worker's compensation experience preferred.
Job Description:
- Serve as a Discharge Planner Case Manager, working day shifts with a business casual dress code.
- No on-call requirement; participate in weekend rotation every 5th or 6th weekend and holiday coverage as needed.
- Responsibilities include coordination of patient discharge, admission criteria review, and care coordination.
- Conduct needs assessments and manage patient placement in home health, hospice, LTAC, nursing homes, and rehab facilities.
- Experience with discharge planning for geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, and MICU.
- Charting experience on Cerner platform required.
- Ensure compliance with CMS, HIPAA, JCC guidelines, and regulations.
- Must provide proof of flu vaccination as per facility requirements.
- Module completion is necessary pre-start and potentially during orientation, with an average of 12 hours to be completed as transcripts.
- Orientation includes 28 hours, with 50% non-billable.
Note: Candidates must be willing to undergo background checks and provide professional references upon request.
