Purpose:
We are seeking a highly motivated and knowledgeable Clinical Pharmacist to join our specialty pharmacy team, focusing on rare disease management. The Clinical Pharmacist will play a critical role in optimizing medication therapy, providing clinical support, and ensuring seamless coordination of care for patients with rare and orphan diseases. This role requires a patient-centric approach, strong clinical expertise, and a passion for improving access to specialized treatments.

This job operates in a professional office environment and teleworking from the employee's home address listed in their employment file. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. When telecommuting employees must have reliable internet access in order to access required systems and software associated with the position's responsibilities. The amount of time the employee is expected to work per day or pay period will not change as a result of participation in the teleworking program. The Clinical Pharmacist will play a critical role in optimizing medication therapy, providing clinical support, and ensuring seamless coordination of care for patients with rare and orphan diseases. This role requires a patient-centric approach, strong clinical expertise, and a passion for improving access to specialized treatments.
Responsibilities:
- Provide therapy dispensing and comprehensive clinical services for patients with rare diseases.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and optimize individualized treatment plans.
- Educate patients and caregivers on medication use, potential side effects, and adherence strategies.
- Monitor patient outcomes, assess treatment efficacy, and make clinical recommendations to optimize therapy.
- Ensure compliance with specialty pharmacy protocols, regulatory guidelines, and best practices.
- Work closely with pharmaceutical manufacturers, payers, and patient assistance programs to facilitate medication access.
- Support prior authorization processes and assist in navigating insurance barriers to ensure timely treatment initiation.
- Maintain accurate and complete documentation of clinical interventions and patient interactions.
- Stay up-to-date with emerging therapies, clinical guidelines, and advancements in rare disease treatment.
- Provide education and training to pharmacy staff, healthcare providers, and patients on rare disease management.
- Supervise pharmacy technicians throughout the prescription fulfillment process.
- Provide on call support services to patients and healthcare providers outside of normal business hours.
Required Qualifications:
- Doctor of Pharmacy (PharmD) degree from an accredited pharmacy school
- Active pharmacist license in good standing with the Pennsylvania State Board of Pharmacy
- Minimum of 3-5 years of experience in a specialty pharmacy setting, with a focus on rare diseases preferred
- Strong knowledge of specialty pharmacy operations, including workflow design, systems setup, and program implementation.
- Excellent communication, organizational, and leadership skills
- Experience supervising and training pharmacists and pharmacy technicians
- Ability to work effectively in a fast-paced, dynamic environment and manage multiple priorities.
- Strong commitment to patient care and quality assurance
- Must be willing to obtain and maintain additional Pharmacist State licensure(s) as requested
- High level of independent judgment, individual initiative, and time management skills
- Ability to communicate effectively in a manner that is clear, logical and consistent
Preferred Qualifications:
- CPR+ or CareTend pharmacy system experience
- Multi-state pharmacist licensure
- Experience managing and leading pharmacists and pharmacy technicians in a specialty pharmacy setting
