Job Description
Job Title: Travel Licensed Practical Nurse (LPN)

Job Specialty: Home-Hospice Care Operations

Job Duration: 13 weeks

Shift: 8-hour Day Shift (40 hours per week)

Guaranteed Hours: Not specified

Experience: Minimum of 3 years of LPN experience in Home Health

License: Active LPN license (State-specific or compact)

Certifications: Active Basic Life Support (AHA), Driver's License, and Car Insurance

Must-Have:
- Experience with wounds, IVs, ostomies, catheters, and drains
- Previous travel experience

Job Description:

- Expectation to see 6-7 patients per day with a focus on common care types such as wounds, ostomies, and IVs.
- Support from a team of 8 Registered Nurses (RNs) and 2 Licensed Practical Nurses (LPNs) if needed.
- Utilize EPIC/EPIC Remote systems for patient care management.
- Any scrub color is acceptable.
- Completion of 1-2 days of orientation is required at the designated home health office.
