Job Description
Purpose:
Under general direction, exercises wide latitude in the use of independent judgment and initiative in the administrative direction and coordination of major activities or functions of setting policy for case management and social work sector, and plans programs for medical and psychiatric teams, community liaison, education, training, research and patient discharge. Implements departmental policies, plans and practices to support effective health, medical and psychiatric care for case management and social work. Acts in and ensures compliance with applicable NYC Health + Hospitals policies and procedures and local, state and federal laws, rules and regulations.
Specific Duties and Responsibilities:
The Director of Social Work Level 6 is charged with and responsible for:
Administration and coordination of the daily activities of the Utilization/Case Management Department. This includes the supervision of professional staff and non- professional support staff.
Administrative oversite of Social Services Department.
Responsible for the timely coordination of on-site review activities of the external review agencies in order to maximize payment of third-party payor, i.e. Medicare, Medicaid,
NYSQRA, Blue Cross NYCHSRO and others.
Prepares monthly reports for the Directors of Service related to the outcome of concurrent quarterly improvement monitoring of departmental indicators.
Prepares quarterly agenda for Utilization Review Committee with input from the Utilization Review Chairman, including the quarterly minutes and annual Utilization Review Program Evaluation.
Liaison to Administration, Medical Records, Patient Accounts, Medical Services, Data Processing and other Hospital Departments.
Review Performance appropriateness and completion of the Case Management Program through random monthly review of the staff's performance followed by individual meetings.
Responsible for the monthly Utilization /Case Management Statistical reports.
Develop and maintains the Utilization/Case Management Department Policy and Procedure Manual.
Responsible for the maintaining the PRO/PC program for tracking and monitoring of data related to review activities for Medicare, Medicaid, Blue Cross and Private Insurance.
Prepares quarterly report for HHC Board Report.
Responsible for the administration of the non-site Home Care Program and supervision of the on-site Nurses from the Certified Home Care Agencies.
Prepares annual UM Program package for distribution to CIH Chairs and HHC.
Member of the Utilization /Case Management Performance Assessment and Improvement
Committee.
Utilization Review Liaison to Managed Care Program with HMO's.
Conducts any other activities as assigned by AED.
Minimum Qualifications
1. Valid license as an LMSW (Licensed Master Social Worker) or LCSW (Licensed Clinical Social Worker) issued by the New York State Education Department (NYSED); and eight (8) years of experience in social work activities, of which four (4) years must have been in a health or medical setting, including psychiatric social work, and four (4) years must have been in a supervisory, consultative and/or administrative capacity in a social work organization, health or medical care setting, or organization adhering to acceptable standards; or
2. A satisfactorily equivalent combination of training and experience. However, all applicants must possess a valid Social Worker license, as described in "1" above.

- Represents the health care setting as a participating educator of other professional disciplines and personnel of health and mental health settings.
- Maintains ultimate responsibility for delivery of social work services involving direct services to patients and their families, consultative services to health care setting staff and Physicians, and consultative relationships with community programs.
- May act as a consultant on discharge planning.
- Performs other related duties, as directed.
