About Us: Meals on Wheels Orange County is the largest nonprofit provider of nutrition and supportive services for at-risk older adults in Central and North Orange County. Our mission is to combat hunger and loneliness among older adults through nutritious meals, friendly visits, and safety checks, and to support families through day services. We deliver nearly 2 million meals annually to 20,000 at-risk older adults.
Location: Anaheim, CA
Hours: Monday - Friday, 7:00 AM - 4:00 PM
Hourly Range: $38.00 - $44.00 per hour (Hourly rate will be determined based on factors such as geographic location, skills, education, and experience)

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Performs assessment of the participants and develops a written care plan. Determines participants needs for personal care, assistance in activities of daily living, such as bathing, grooming, toileting and eating. Maintains client care plans.
- Assess, re-assesses and updates care plans every six months on all participants.
- Administers medication to participants
- Supervises ADHC Nursing Team; Licensed Vocational Nurse "LVN", Certified Nurses Assistant "CNA", and Program Aides.
- In conjunctions with Program Director planning nursing team assignments, schedules, ensuring adequate coverage and efficient workflows
- Maintains participants' health records, which includes documentation of all skilled nursing care provided on the day the service is given.
- Completes and signs quarterly progress notes indicating participants' progress towards achieving health goals.
- Notifies the physician of any marked changes in participants' condition.
- Accepts telephone orders from participants' personal physician. Obtains written orders and documents in chart.
- Provides supportive and restorative nursing as indicated by the treatment plan.
- Maintains medical supplies and participant medications according to regulations and center procedures.
- In consultation with the Program Director, establishes and monitors standards of sanitation, cleanliness and health & safety in the center.
- Provides emergency nursing care
- Assists with referral screening process. Reviews required documentation for potential program participants.
- Maintains regular consultation with all MDT members; PT, OT, RD, LCSW and MD
- Supervises therapy activities and maintains regular consultation with Physical and Occupational Therapists.
- Ensuring adherence to ADHC/CBAS policies and procedures
- Member of the Multidisciplinary Assessment Team.
- Performs other tasks as necessary.
EDUCATION, EXPERIENCE AND QUALIFICATIONS:
- Graduate of an accredited school of nursing with at least two years' skilled nursing experience.
- Must be registered and currently licensed in the State of California by the California Board of Registered Nursing.
- Must have current certification in CPR and first aid training.
- Experience working with the elderly or disabled adults.
- Public Health Nurse Certification desired.
- Good clinical and communication skills and the ability to work as a team member.
- Must pass pre-employment physical and obtain tuberculosis clearance.
