The role youll contribute:
The Clinical Pharmacist Transplant is a dedicated member of the transplant team. Provides comprehensive pharmaceutical care to transplant recipients and donors. Will work with patients and their families, and members of the transplant team, including physicians, surgeons, nurses, clinical coordinators, social workers, financial coordinators and administrative personnel to coordinate care throughout all phases of the transplant process including discharge planning. Will attend and participate in all selection committee meetings and inpatient rounding with multidisciplinary team.

- Defines pharmacy services to be provided to the transplant patients and care giver/family of all solid organ transplant services during the pre-op, peri-op, inpatient, discharge and post-op phases of transplantation or donation.
- Maintain documentation related to transplant pharmacy activities that are compliant with CMS regulations.
- Clinical pharmacy services will be provided for kidney, liver and combination transplant patients as needed across the continuum of care.
- Identifies, solves and prevents medication related problems in the solid organ transplant population and living donor for the kidney and liver program.
The expertise and experiences youll need to succeed:
Required qualifications:
- PharmD degree
- A minimum of one year of experience as a clinical pharmacist
- Previous experience in Transplant Pharmacotherapy
- Experience with IV Additive Preparation and Unit Dose Dispensing
- Current Pharmacist license in the state of Colorado
Preferred qualifications:
- Completion of a Pharmacy Practice Residency (PGY-1) or Residency in Transplant and/or Critical Care (PGY-2), preferred. Board Certified (BCPS)
- PET
