Summary
The Patient Safety Coordinator acts as a consultant for the event reporting system at both departmental and organizational levels. Responsible for supporting the Patient Safety Program to safeguard patients from harm, and devising strategies to prevent future incidents through process improvements and staff training.
Responsible for managing patient safety activities, including analyzing near misses and adverse events, conducting educational sessions, assisting with clinical initiatives, and ensuring compliance with regulatory standards to enhance and promote patient safety and quality care.
Keeps up to date with trends and initiatives related to patient safety. Supports patient safety initiatives through guidance and active involvement. Serves as a resource, internal consultant, and educator on patient safety issues. Develops and implements programs to minimize the frequency and severity of actual and potential patient safety hazards throughout the facility. Engages in patient safety activities with other clinical disciplines. Actively participates in Root Cause Analysis (RCA) and provides recommendations for improvement. Actively participates in Failure Mode and Event Analysis (FMEA). Leads investigations into serious adverse events. Serves on Hospital Committees, as assigned.
Position Reports To: Director of Risk Management
Essential Functions
Reviews, enters, and edits risk/patient safety events to ensure they are complete, accurate and triages these events to key stakeholders. Facilitates the event review process, leading committees to reach consensus on root cause(s) and action planning. Monitors event reporting system changes and keeps end users informed as appropriate. Educates, trains, and mentors others on aspects of the reporting system and patient safety standard work. Communicates effectively with team members, providers, and others. Monitors shared email communications and provides timely responses. Designs and delivers safety messaging. Utilizes project enhancing tools for process improvements, problem analysis, and measurement to lead projects. Performs Root Cause Analysis and implements corrective actions, ensuring event resolution and, if required, reporting to the Washington State Department of Health. Prepares cases for RCAs, gathering additional information from relevant departments and care providers. Identifies Good Catch/Near Miss event recipients and schedules recognition as appropriate Actively participates in safety management and planning with the Confluence Health Safety Committee. Documents, trends, and plans for remediation of situations that present risks to patient safety. Evaluates incident reports using the Confluence Health Reporting system and conducts trend analysis. Attends required meetings and serves on committees as requested. Promotes a culture of safety within the organization. Performs other duties as assigned.

Qualifications
Required:
- Current RN licensure in the state of Washington (RCW 18.88) or licensure through Multistate Nurse Licensure Compact (SSB 5499)
- Bachelor's degree in healthcare related field
- A minimum of 2 years of clinical experience in a hospital or healthcare setting
Desired:
- Certified Professional in Healthcare Quality (CPHQ)
- Certified Professional in Patient Safety (CPPS)
- 2 years of related experience in patient safety, quality, or healthcare related process improvement
- Experience with RLDatix reporting system and EPIC
