Job Description
Under general direction, formulates health care facility policy in the social work sector, plans programs for medical and psychiatric social work community liaison, education, training, research and patient discharge. Implements social work policies, plans and practices to support effective health, medical and psychiatric care.
Establishes Social Work Department policies, methods and procedures, program assignments and development. Determines program priorities within the scope of program planning. Develops, plans and administers social work programs to support patient care. Coordinates social work programs and assures that programs are related to health care facility and Corporation's objectives in support of effective health, medical and psychiatric care. Collaborates with administration and professional staffs in planning social work participation within the health care facility, in education and training programs and in formulation of policy and practices in accordance with objectives of the health care facility. Develops communications programs to achieve the appropriate interpretation of the medical and psychiatric social work program. Develops department budget in conjunction with health care facility administration; recommends and implements. Organizes the personnel of the department into areas of responsibility, authority and specialization. Staffs, directs, guides and controls departmental activity. Evaluates personnel performance and departmental work progress. Establishes departmental work standards and practices. Provides leadership to attain effective productivity and initiates action to accomplish objectives of Social Work Department. Develops, establishes and administers training programs for social personnel and in-service training for professional and non-professional personnel for social work activity in support of patient care. Participates in formulation and development of health care facility policies regarding the participation in community planning with respect to social work and psychiatric social work service. Provides consultation to community agencies regarding use of health care facilities and activities relating to medical and psychiatric social work. Formulates, plans, directs and initiates research objectives and programs in areas of social work and coordinates cooperative research projects with other departments. Evaluates and audits social work programs and program accomplishments. Makes recommendations for appropriate action. Maintains relationship with professional societies in the field of social services, social work and related disciplines. Keeps abreast of new developments and approaches, with particular reference to health, medical and psychiatric care fields. Evaluates and makes appropriate recommendations for improvement of social work. Represents the health care facility as a participating educator of other professional disciplines and personnel of health and mental health facilities. Maintains ultimate responsibility for delivery of social work services involving direct services to patients and their families, consultation service to hospital staff and Physician and consultation relationship with community programs. Participates in discharge planning. Assists the finance team with following up to families on open financial questions. Ensures team's timely completion of documentation elements (e.g., electronic medical records) including but not limited to admission information, assessments, care plans and participation in interdisciplinary care plan meetings. Participates in bioethics discussions. Ensures advance directives are accounted for in a timely manner. May provide tours to prospective residents and families. Actively participates in H3 program. Facilitates patient satisfaction surveys. Communicates with families through phone calls, Outlook e-mail and mass e-mails through the PCC EMR system. Communicates with the Ombudsman as needed. Sends information to the Ombudsman regarding transfers out of the facility. Acts as an ambassador of the facility. Serves as the facility Grievance Officer. May assist residents/families with benefit information, guardianships, identity information. Compiles and provides data to Administration, the Nursing departments and Central Office as needed. Involved with quality initiatives.
Minimum Qualifications
1. Valid license as an LMSW (Licensed Master Social Worker) or LCSW (Licensed Clinical Social Worker) issued by the New York State Education Department (NYSED); and eight (8) years of experience in social work activities, of which four (4) years must have been in a health or medical setting, including psychiatric social work, and four (4) years must have been in a supervisory, consultative and/or administrative capacity in a social work organization, health or medical care setting, or organization adhering to acceptable standards; or
2. A satisfactorily equivalent combination of training and experience. However, all applicants must possess a valid Social Worker license, as described in "1" above.
