Job Description:

- The unit comprises 36 beds and 2 staff members, including 1 RN and 1 Social Worker during the day shift.
- Manage patient ratios with 1 RN per 25 patients.
- Primary responsibilities include discharge planning, care progression, and communication with medical teams.
- Ideal candidates will have experience in discharge planning and patient progression.
- The role may require floating to Med/Surg and ICU areas.
- Dress code is business casual or navy scrubs.
- Required to work one weekend a month with rotating holidays.
- Best suited for candidates who are open-minded and proactive.
