The Community Mental Health Service at Unity Health Toronto's St. Michael's site provides community outreach-based recovery oriented and trauma informed services to people living in Southeast Toronto who require intensive case management services and psychiatric follow up support. Clients receive community-based assessment, supportive counselling, linkage with community resources, crisis prevention and intervention support, psychoeducation, skill teaching, family support and education, medication management and monitoring, individual and system level advocacy.

The Operations Leader, Community Mental Health Program is a Master's Prepared Health Disciplines professional (Social work or occupational therapy) with progressively responsible work experience in community mental health services. The Operations leader holds subject matter expertise on the implementation of the community mental health models of care, recovery-oriented and trauma informed care and implementation of evidence-based brief structured treatments for people with schizophrenia and bipolar disorder. The Team Leader provides operational, technical and clinical expertise to the Community Connections Intensive Case Management Team , STEPS for Youth Early Intervention in Psychosis team, Project Dignify Intensive Case Management Team for older adults and Beyond Housing a wellness program for homeless persons living in shelters.

DUTIES & RESPONSIBILITIES

  • Identifies clients with complex care needs who meet MOHLTC criteria for collaborative care planning
  • Chairs collaborative care planning meetings with clients, families and community agencies in attendance
  • Provides clinical consultation to team members to support the development of recovery-oriented community-based care plans
  • Promotes effective transitions in client care between hospital, community and legal system
  • Represents the Mental Health and Addictions Service on internal and external committees that focus on improving care experiences for people with mental disorders and/or improving health/social system integration
  • Provides monthly 1:1 coaching and mentoring sessions for interprofessional clinicians to promote ongoing practice development and implementation of evidence-based treatments
  • Coordinates access and flow to community mental health services
  • Tracks and trends intake and discharge data and uses data analytics to inform decision making to improve access to care for people living with severe mental illness
  • Coordinates and facilitates new staff and student onboarding
  • Works collaboratively with clinical teams to improve clinical processes, policies and procedures
  • Supports level of care decision making and stepped care for program clients
  • Assigns new clients to interprofessional clinicians and psychiatrists on a rotational schedule
  • Advocates for discharge from the to a more appropriate level of care as required
  • Chairs interprofessional clinical team meetings
  • Works collaboratively with team members to problem solve and resolve clinical issues/challenges
  • Works collaboratively with the Clinical Leader/Manager to develop and implement strategies to improve work flow processes to increase efficiencies in workload and service delivery
  • Collects data, prepares and submits statistical reports to meet MOHLTC requirements
  • Participates in quality improvement projects being implemented in community mental health services
  • Participates in Mental Health and Addictions Service and hospital-wide committees as required

QUALIFICATIONS:

  • Regulated Health Professional designation with registration with the Ontario College of Social Workers, College of Occupational Therapists of Ontario or College of Nurses of Ontario
  • Minimum five years of full-time experience working in a clinical capacity in a community mental health program
  • Demonstrated experience providing recovery-oriented mental health and addictions care to individuals living with mental illness and/or addictions
  • Previous experience coaching and mentoring students and/or staff preferred
  • Excellent knowledge of the community mental health models of care and standards of care
  • Excellent knowledge of the values, principles and practices of recovery-oriented care
  • Very good knowledge of the social determinants of health as well as the challenges faced by disadvantaged populations
  • Excellent communication and interpersonal skills
  • Excellent oral presentation and writing skills
  • Very good conflict resolution and mediation skills
  • Sound time management and organizational skills
  • Familiarity with quality improvement science is preferred
  • Excellent punctuality and attendance record
