Job Description
The System Lead Clinical Pharmacist Specialist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Key responsibilities include standardization of necessary aspects of the medication-use system to ensure comprehensive and individualized pharmaceutical care to our patients. System Lead Clinical Pharmacist Specialist serves as departmental resource and liaison to other departments, hospital personnel, or external groups. System Lead Clinical Pharmacist Specialist develops practice advancement projects and patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. System Lead Clinical Pharmacist Specialist provides medication and practice-related education/training to all pharmacy, nursing and physician staff members as needed. As applicable, participation in the quality management program is expected with the goal of improving services by monitoring processes, analyzing data, implementing interventions to improve, and evaluating the effectiveness of those interventions. Responsibilities may include working to establish and maintain long- and short-term goals for the Quality Management Program; monitoring and documenting Quality Improvement Projects for progress in meeting QI goals; and providing guidance and education to staff on Quality Management priorities and projects.
ACCOUNTABILITIES
1. Supervises clinical pharmacists in the acute care setting. Assists/coordinates with clinical pharmacists in the ambulatory setting.
2. Coach and provide feedback to clinical pharmacists and other staff on areas for improvement.
3. Perform annual evaluations and assure competencies are completed for clinical pharmacists.
4. Work with pharmacy directors to develop competencies for front line staff (pharmacists and technicians)
5. Support and grow pharmacy residency programs as applicable.
6. Define clinical pharmacy practice and create metrics to monitor clinical impact to patient care.
7. Provide education to pharmacy and hospital staff.
8. Lead formulary and pharmacy & therapeutics committees and leads formulary review as part of these committees.
9. Provider education, specifically regarding formulary changes and initiatives.
10. Design programs to integrate and optimize utilization of pharmacy and medical learners.
11. Implement evidenced based practice guidelines.
12. Collect pharmacy quality data and report to appropriate groups.
13. Collaborate with physicians to improve patient care outcomes.
14. Collaborate with 340b and procurement teams to assure safe, cost-effective medication use.
15. Collaborate with ambulatory care pharmacy clinicians to align strategies.
16. Other duties as assigned.
Job Requirements
REQUIRED QUALIFICATIONS
Education: Bachelor's degree in Pharmacy.
Skills: Proven communication skills and ability to communicate effectively with all levels of personnel. Demonstrated knowledge of electronic pharmacy applications.
License: Pharmacist licensure in current state of practice.
Certification: Certification required to meet ASHP preceptor requirements.
PREFERRED QUALIFICATIONS
Education: Doctor of Pharmacy Degree.
Years of Experience: Five years of experience as a pharmacist in an acute care hospital. Residency required for precepting residents.
