Type: Full-Time, Rotating Weekends Job Summary: Mount Sinai Hospital is seeking a dedicated and detail-oriented Clinical Pharmacist to join our inpatient pharmacy team. This full-time position with rotating weekend coverage plays a vital role in ensuring safe and effective medication use, optimizing patient outcomes, and providing direct clinical support to healthcare teams.

As a key member of the interdisciplinary care team, the Clinical Pharmacist will be responsible for medication order verification, participation in patient rounds, therapeutic drug monitoring, and implementation of evidence-based pharmacotherapy recommendations. The ideal candidate will have strong clinical judgment, excellent communication skills, and the ability to work efficiently in a fast-paced hospital setting. Key Responsibilities: Review and verify medication orders for accuracy, appropriateness, and adherence to hospital policies and clinical guidelines. Provide pharmacokinetic monitoring and dose adjustments for high-risk medications (e.g., antimicrobials, anticoagulants, and narrow therapeutic index drugs). Participate in interdisciplinary rounds, collaborating with physicians, nurses, and other healthcare professionals to optimize patient care. Conduct medication reconciliation and patient education to improve medication adherence and reduce adverse drug events. Support antimicrobial stewardship initiatives by evaluating and recommending appropriate antimicrobial therapy. Ensure compliance with hospital protocols, regulatory standards, and formulary management strategies. Provide pharmacy consulting services for drug information, adverse event monitoring, and medication therapy management. Mentor and support pharmacy residents and students as part of their clinical training.

Required Qualifications: Doctor of Pharmacy (PharmD) from an ACPE-accredited institution. Active Illinois pharmacist license in good standing. Previous hospital or inpatient clinical pharmacy experience required. Strong knowledge of EPIC, Omnicell, and clinical decision-support tools. Ability to work rotating weekends as part of a full-time schedule. Preferred Qualifications: Completion of a PGY-1 pharmacy residency or equivalent clinical experience.

Board Certification (e.g., BCPS) preferred. Experience with sterile compounding, USP 797/800 compliance, and investigational drug services. Why Join Us?
