Job Description
Job Title: Discharge Planner Case Manager
Job Specialty: LCSW (Licensed Clinical Social Worker)
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 16:30)
Guaranteed Hours: Not specified
Experience: A minimum of 2 years in discharge planning including admission criteria review, benefits eligibility, care coordination, conducting needs assessments, and ordering DME. Experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, and acute hospital experience is required.
License: Must be eligible for LMSW/LGSW/LCSW with exam passed, or have pending licensure with proof of application.
Certifications: BLS required
Must-Have: Compliance with CMS, HIPAA, JCC regulations. Workers' compensation experience preferred.
Job Description:
- Manage patient discharge planning, ensuring efficient and safe patient transitions.
- Conduct needs assessments for patients and coordinate necessary services and equipment.
- Review admission criteria and benefits eligibility to facilitate appropriate care plans.
- Coordinate with home health, hospice, and other post-acute care providers.
- Maintain accurate patient documentation using Cerner charting system.
- Work collaboratively within a multidisciplinary team to address patient needs.
- Maintain professional appearance in a business casual uniform.
- Comply with mandatory flu vaccination policy.
- Participate in weekend rotation approximately every 5th or 6th weekend, with holiday coverage as needed.
- No on-call or floating requirements.
- Complete orientation and pre-start modules, potentially during the orientation period.
Note: The flu vaccine is required with no exemptions. No requirements for the COVID-19 vaccine. References from two supervisors are required.
