Who you are:
  • Current RN or LVN Licensure by the California Board of Nursing with MDS certification.
  • Degree from an accredited school of nursing, BSN, preferred
  • Supervisory and geriatric nursing experience, a minimum of 2 years preferred
  • Minimum of 1 year of experience completing and submitting MDS
  • Current CPR certification

Job Summary:

Coordinates the completion of comprehensive assessments of residents upon admission, quarterly, annually, and for permanent change of condition under H.C.F.A. guidelines. Coordinates the interdisciplinary team care plan conferences and monitors all resident care plans for accuracy and timeliness of completion.

Essential Job Duties:
  • Develops and revise the MDS/care plan policy and procedure as needed
  • Monitors and interacts with interdisciplinary team members for completion and updating of MDS (Minimum Data Set)
  • Completes MDS on each admission and keeps these current, following OBRA guidelines. Coordinates the automation and electronic transmission of MDS, following state/federal guidelines
  • Participates in contributing to initial care plans and monitors all care plans on an ongoing basis for accuracy, completeness, and timeliness
  • Monitors the output of staff nurses responsible for completing assessments (admission, quarterly, change of condition, and annual)
  • Completes resident assessment as needed
  • Schedules, conducts as needed, and notifies participants of interdisciplinary care plan meetings.
  • Assures follow-up of goals and plans developed at interdisciplinary care plan meetings.
  • Assures that state/federal regulations are followed in regard to resident care and documentation
  • Participates in closely monitoring the use of physical and chemical/psychotropic restraints, encouraging the safest implementation level possible
  • Maintains open communication with all residents, responsible parties of residents, physicians, other staff members, and involved representatives of the community (friends of residents and staff of community agencies)
  • Attends in-service training classes and outside workshops and shares information with the Nursing Staff
  • Participates in Quality Assurance Committee by reporting resident care issues at the quarterly meetings and serves on QA subcommittees as assigned
  • Attends the weekly rehab meetings, nutritional support meeting as needed, and other meetings as assigned

Pay Rate:

$50.00-$60.00/hr.
