Job Title: Registered Nurse

Guaranteed Hours: Not applicable
Experience: Minimum 1 full year Adult Home Health experience or 1 full year Hospice experience, depending on specialty
License: Must have license in hand
Certifications: CPR, BLS
Must-Have:
- Proficiency in OASIS
- Proficiency in HomeCare HomeBase (HCHB)
- Reliable transportation, current driver's license, and auto insurance
- Willingness to travel up to 75 miles from the assigned clinic
Job Description:
- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies and procedures.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Manage a caseload averaging 5-8 patients per day, totaling approximately 6-8 points.
- Handle various types of cases including Start of Care, Wound Care, and Follow-up Visits.
- Perform all documentation within 24 hours of the patient appointment.
- Travel to patient homes and be compensated for mileage at the IRS Standard rate between patient locations.
- Attend weekly meetings and cover the Raleigh and Wyoming counties.
- Complete all documentation promptly to ensure seamless billing and patient follow-up care.
- Achieve weekly productivity points of 30, equivalent to a 40-hour workweek.
- Float to nearby clinics if necessary to maintain financial needs due to low patient census.
- Adhere to a dress code of scrubs and use proficiently the HomeCare HomeBase EMR system.
- Engage in a standard schedule of Monday to Friday, 8:00 AM to 5:00 PM.
