Job Description
Job Title: Licensed Clinical Social Worker
Job Specialty: Social Services - LCSW
Job Duration: 13 weeks
Shift: Monday to Friday, Day Shift, 07:30 AM - 4:00 PM
Guaranteed Hours: Client may call-off up to 36 hours per thirteen-week assignment without compensation for canceled hours. If the clinician is placed on-call instead of being called off, it will count as a called-off shift.
Experience: Minimum 1 year required
License: Must have active LCSW at the time of start
Certifications: LCSW required
Must-Have: Outpatient Clinic experience
Job Description:
- Handle a daily caseload of 5-7 patients.
- Conduct biopsychosocial assessments and Clock Drawing Dementia Screen.
- Collect and analyze patient information and conduct initial evaluations.
- Develop plans of care to meet patient and caregiver needs, including the development of a problem list.
- Establish and revise patient goals, utilize the Geriatric Depression Scale, and conduct Mini-Mental State Examinations.
- Review and document health history.
- Coordinate collaborative care with internal and external team members, participating in care conference activities.
- Document patient care and teaching, reflecting an understanding of coverage criteria.
- Facilitate discharge planning, family therapy, group therapy, and individual counseling.
- Provide crisis intervention and support long-range planning and decision-making.
- Participate in treatment plan development and conduct educational support for patients and families.
- Work within a multidisciplinary team to ensure continuity of care.
Additional Requirements:
- Participate in non-billable modules for up to 16 hours, completed virtually on the first day by end of the day, then annually. Non-clinical hours do not vary; module hours may vary for clinical roles.
- A flu vaccine is required, with medical or religious exemptions only.
- Participate in non-clinical orientation for 16 hours, non-billable.
- Participation in a minimum of two out of three major holidays.
Candidates must ensure housing arrangements and complete all compliance needs in a timely manner, committing to the full assignment without attendance or behavior issues.
