RN Case Manager
The PACE RN Case Manager promotes and maintains the health of participants in the community through teaching, counseling, and coordination of appropriate preventive and rehabilitative services with the PACE interdisciplinary team. The RN Case Manager is responsible for assessment of health needs and oversight of the plan of care, initial implementation of nursing care plans and providing nursing care, as well as periodic re-evaluation of individual and family nursing needs. This role provides technical, organizational, and interpersonal skills necessary to coordinate the scheduling, communication, and encounter documentation of all aspects of the plan efficiently and effectively for home care. The RN Case Manager oversees and updates the CNA plan of care.

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.
The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

What You Bring:
- RN with current Colorado licensure, maintained throughout employment.
- Associate's degree in nursing required, bachelor's degree preferred.
- At least 3 years of recent experience in a hospital or clinic setting preferred.
- A genuine spirit of caring and compassion
- Valid Colorado driver's license, adequate automobile insurance and access to reliable transportation if working in the community setting.
- Clinical expertise applicable to the PACE setting.
- Excellent interpersonal and communication skills.
- Current CPR certification is maintained throughout employment.
- Adequate computer literacy in MS Word, Excel, Access, Outlook
- Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.
