Are you a skilled and compassionate Registered Nurse looking for a meaningful role in home health care? Beebe Healthcare is seeking an RN Admissions Nurse to be the first point of contact for patients transitioning to home-based care. In this pivotal role, you will conduct admissions and OASIS assessments, ensuring each patient's plan of care is tailored to their unique needs.

As a key member of our multidisciplinary home health team, you will provide skilled nursing care in collaboration with physicians and under the guidance of a Clinical Supervisor. Your expertise will help ensure seamless transitions, optimal patient outcomes, and a compassionate experience for those receiving care in the comfort of their homes.

Responsibilities

  • Assessing and admitting eligible patients for home health care services.
  • Evaluation, development and implementation of plan of care.
  • Skilled Nursing Care and Comprehensive Assessment.
  • Teaching, Supervision, and Counseling Clients and Caregivers.
  • Interdisciplinary collaboration.
  • Problem solving and critical thinking skills.
  • Oasis documentation.
  • Provides appropriate care for a range of physician-ordered treatments (PleurX Drain, Finger stick Coagulation checks, wound care, wound vacs, IVs, phlebotomy, Trachs, Peg Tube, Ostomies, Telehealth)
  • Other duties as assigned.


Qualifications

This position is 10-hour days 4 days a week. You will work 1 weekend / month and two holidays/year

Requires at least 1 year of RN Home Health experience

* Experience In Hospital Or Acute Care Setting
* Clear Communication Skills Both Written And Verbal
* Able To Keep Confidential Information Regarding Patients, Team Members
* Able To Withstand Crisis Situations
* Has Skills To Provides Customer Service To Patients, Team Members And Visitors
* Proof Of Current/Valid Car Insurance
* Knowledge And Experience With Electronic Health Records
* Valid DL With No More Than 3 Moving Violations

Credentials

Essential:
* RN - Registered Nurse
* BLS - Basic Life Saving certification

Education

Essential:
* RN Diploma/RN Graduate Program
