Job Description
Purpose of Position:
Under the direction of the collaborative clinical team lead, performs clinical services relating to the review, evaluation and management of drug therapy to a patient, being treated by a physician for a specific disease or associated disease states, in accordance with a written agreement or Protocol with a voluntarily participating physician.
Examples of Typical Task:
1. Operates under defined protocols based upon agreement(s) with Collaborating Provider(s) and in accordance with the policies, procedures and protocols of NYC Health + Hospitals (H+H) and New York State (NYS) guidelines.
2. Implements and modifies drug therapy of individual patients or groups of patients (patients with diabetes, asthma, hypertension, etc.).
3. Adjusts the management of drug strength, frequency or route of administration and/or substitutes authorized drugs in accordance with the Drug Therapy Protocol with the Collaborating Provider.
4. Evaluates clinical laboratory tests related to the drug therapy management for the specific disease or disease state specified within the Protocol and orders such clinical laboratory tests, only if specifically authorized by the protocol and to the extent necessary to discharge their responsibilities under the law.
5. Orders and/or performs routine patient monitoring functions, as necessary, in drug therapy management including collecting and reviewing patient histories, conducting relevant physical assessments and ordering or checking of patient vital signs, including pulse, temperature, blood pressure and respiration, to the extent necessary to discharge responsibilities set forth in the Protocol.
6. Documents modifications made to the patient drug therapy in each participating patient's record and notifies other treating providers, as required.
7. Performs comprehensive medication review and reconciliation and resolves medication-related problems in order to optimize drug therapy.
8. Provides patient education on self-management and assists patients and providers with medication access issues to ensure understanding of the care plan.
9. Performs related duties, as directed
Responsibilities:
Know the contents of the NYC H+H Drug Formulary, inclusive of restricted agents and required ordering forms (e.g. the non-formulary drug request form).
Be familiar with the standards of patient care; understand the medical conditions, treatment goals, treatment drugs & protocols.
Fill prescriptions and patient orders, resolves discrepancies inpatient therapy, and follows-ups on problems.
Interview (meet) the patient. Conduct a medication therapy review and communicate with the physician/care team as needed in order to provide comprehensive patient care. Therapy review and the patient interview may include the following but is not limited to:
o Introduction to the patient and explanation of the role of the Pharmacist on the Team.
o Confirm the name and address of the pharmacy most utilized by the patient.
o Interview the patient to gather all relevant data, including but not limited to general health, activity status, medical history, medication history (including OTCs), and immunization history.
o Improve compliance by coaching patients to manage their medication therapy as prescribed.
o Conduct a medication reconciliation comparing medications the patient was on and incorporating any changes in regimen or instructions and communicate same to the patient / care giver
o Discuss with the patient the Pharmacist-expected interactions (any follow-up contact)
o Based on available clinical information, assess the patient's overall health status values, preferences, quality of life and goals of therapy.
o Evaluate the patient to detect symptoms that could be attributed to adverse events caused by his/her current medication(s).
o Identify and prioritize medication related problems.
o Develop a plan for resolving medication related problems.
o Interpret, monitor and assess the patient's laboratory results
o Monitor the patient's response to therapy including safety and effectiveness.
o Communicate appropriate information and clinical concerns/interventions to the patient's physician.
o Communicate Medication list to the patient's provider(s)
o Follow-up phone calls to patient, patient's caregiver and/or pharmacy and/or to confirm compliance with filling prescriptions/ compliance with medication regimen and understanding ofinstructions.
Prepare written notes (document) in a consistent manner on all patients seen. Enter progress notes (documentation) in the electronic medical record
Work collaboratively and help coordinate activities with the care team to develop the patient care plan, promote adherence to treatment plan and encourage appropriate follow-up for optimal medical management.
Participate in care team meetings
Recommend dose adjustments when required based on patient renal, liver function and other lab results.
Monitor and document and report adverse drug reactions (ADRs)
Check for patient drug allergies and update all computer information.
Support/ assist patient or caregiver to obtain prescriptions; confirm prescription insurance coverage, formulary status and ability of patient to obtain medications
Provide drug information to Medical and Nursing staff upon request and as needed.
Perform other related duties as assigned
Minimum Qualifications
1. A valid New York State license and current registration to practice pharmacy; and
2. Must be certified to practice Collaborative Drug Therapy Management (CDTM) by the New York State Education Department (NYSED).
