Description:
Under the general supervision of the Director of Pharmacy Services, the Clinical Pharmacist assumes responsibility and accountability for managing rational antibiotic therapy within White Plains Hospital or overseeing critical care and high-risk patients as defined by the area of responsibility.
The Clinical Pharmacist collaborates and consults with other healthcare professionals regarding the therapeutic use of antibiotics or other medications for patients within their scope of responsibility. Additionally, they provide education and training for healthcare professionals involved in this clinical forum.
Essential Functions and Responsibilities
Understands and adheres to the client Performance Standards, Policies, and Behaviors.
Improves the monitoring and management of the clinical pharmacy antibiotic service, or the care of critical care and high-risk patient groups, through interventions that enhance the quality and cost-effectiveness of patient care.
Promotes clinical pharmacy development by providing leadership and education for pharmacy staff.
Works collaboratively with colleagues to achieve organizational objectives, fostering teamwork and encouraging new ideas.
Generates creative approaches and innovative solutions to improve performance, service, and quality, involving appropriate stakeholders in decision-making.
Takes personal responsibility for all work activities, follows through on commitments, implements agreed-upon decisions, and acknowledges mistakes without placing blame on others.
Demonstrates understanding and sensitivity toward others by treating people with courtesy and respect, maintaining positive relationships, and fostering a welcoming environment.
Displays a high degree of honesty, loyalty, and integrity.
Builds trust and collaboration among colleagues and departments by aligning work goals with the hospital's mission and values.
Maintains open communication by listening attentively and responding positively to requests, questions, and suggestions.
Shares information in a timely manner and ensures others are adequately informed.
Practices cost-containment measures.
Adheres to all client policies and procedures, including dress code, behavioral standards, conduct, customer service expectations, mandatory in-services, and attendance requirements.
Performs all other related duties as assigned.
Education & Experience Requirements
Graduate from an accredited school of Pharmacy with a PharmD degree.
Current New York State Registration.
