We are looking for a dedicated and experienced Nurse Practitioner to join our clinical team in Burbank and Glendale, CA. In this role, you will provide in-home health care services, conduct assessments, and deliver compassionate patient care within your assigned territory.

Position Overview:
We are looking for a dedicated and experienced Nurse Practitioner to join our clinical team in Burbank and Glendale, CA. In this role, you will provide in-home health care services, conduct assessments, and deliver compassionate patient care within your assigned territory.
Key Responsibilities
- Home Visits:Conduct 5-7 home visits daily for patients with complex care needs.
- Health Assessments:Perform comprehensive health evaluations, including:
- Detailed medical history and physical examinations
- Medication reviews and safety assessments
- Evaluation of home safety and psychosocial needs
- Documentation:Accurately document all pertinent patient information, including medications, end-of-life preferences, and factors contributing to preventable hospital admissions on the same day as visits.
- Patient Guidance:Educate patients and their families to contact you before seeking medical care (except in emergencies).
- Safety Compliance:Maintain patient safety and confidentiality while adhering to universal precautions in all care activities.
- Prescribing:Prescribe treatments and medications within your scope of practice.
- Collaboration:Rotate with the inpatient hospitalist team as determined by the Campus Medical Director to enhance clinical skills.
- Post-Discharge Care:Evaluate patients at the Post Discharge Clinic alongside the attending physician.
- Relationship Building:Foster trust and strong relationships with patients and their families.
Compensation and Benefits
- Salary Range:$135,000 - $145,000 annually (actual salary may vary based on experience, location, and qualifications).
- Total Compensation Package:Includes potential sign-on bonuses and discretionary awards.

Qualifications
- Language Skills:Bilingual in Armenian is required.
- Experience:Minimum of 1 year of relevant experience, preferably in home health care.
- Education:Graduation from an accredited Nurse Practitioner program.
- Certification:Current Board Certification as a Nurse Practitioner.
