Schedule/ Shift:
- Full-time: Monday-Friday, 8:30 AM - 5:00 PM or
- Part-time: Wednesday and Thursday, 8:30 AM - 5:00 PM (potential for weekend Urgent Care hours)
Job Description
Responsibilities/ Job Duties:
- Triage patients by telephone or in person to assess urgency and determine the appropriate level of care.
- Perform patient procedures, assess health status, and provide health education regarding clinical conditions, treatments, follow-up actions, and home care.
- Communicate abnormal test results to providers.
- Track patients for necessary follow-up and document appropriately.
- Collaborate with MAs, providers, and the SUD team to address patient concerns and ensure comprehensive care.
- Maintain accurate patient records and support overall team efforts to deliver patient-centered care.
Minimum Education and Experience Qualification Requirements:
- Massachusetts RN License
- BLS Certification
- Experience working with diverse patient populations preferred
- EPIC experience, bilingual skills, and FQHC experience are a plus
