Job Summary:
The Director of Case Management is responsible for the assessment, planning, coordination, development, and evaluation of the Case Management program. This includes the functions of case management, utilization management, discharge planning, and social services. The role is accountable for assessing and monitoring the care and services provided by the department staff. Additionally, the Director is responsible for staffing levels, competency, and educational needs of the case management staff to ensure the effective coordination and control of resources. The position also involves providing consultation and education to leadership, clinical and financial personnel, and medical staff regarding case management, utilization, and discharge planning practices. Compliance with local, state, federal, and private agencies related to Case Management and Utilization Management is essential. The Director will also provide leadership in fulfilling organizational initiatives and expectations.

Responsibilities:
  • Oversee the assessment, planning, and coordination of the Case Management program.
  • Manage case management, utilization management, discharge planning, and social services functions.
  • Monitor and assess care and services provided by the case management staff.
  • Ensure staffing levels, competency, and educational needs of staff are met.
  • Provide consultation and education to leadership and medical staff on case management and utilization management practices.
  • Ensure compliance with applicable regulations from local, state, federal, and private agencies.
  • Lead initiatives and expectations aligned with organizational goals.

Skills and Certifications:
  • Licensed Social Worker (MSW) OR Graduate of an accredited school of Nursing (BSN) required.
  • Current Social Worker or Registered Nurse license in the relevant state.
  • Minimum of five (5) years of clinical experience required.
  • Minimum of two (2) years of progressive leadership experience in case management or a related field.
