Job Title: Outpatient/Inpatient Social Worker/Registered Nurse (RN) Case Manager
Job Specialty: Licensed Clinical Social Worker (LCSW)
Job Duration: 13 months
Shift: Monday to Friday, 8am-5pm with weekend on-call rotation (1 weekend a month)
Guaranteed Hours: Not specified
Experience: Minimum of 3 years in a clinical care setting
License: Must have a valid, current, full, and unrestricted Professional License to practice LMSW or LCSW in any state, commonwealth, or territory.
Certifications:
- Current Basic Life Support (BLS) certification from the American Heart Association
Must-Have:
- Current immunizations record
- Negative TB skin test proof from the last two years
- Documentation of 2 MMR vaccinations, Tdap, and Flu shot
- Negative 10-panel drug screen within 7 days of start
- Clean background check
Job Description:
- Serve as a Case Manager for designated high-risk patients in both outpatient and inpatient settings.
- Design and manage a continuum of care focusing on empowering clients to achieve demonstrable outcomes and self-sufficiency.
- Responsible for assessment, service planning, resource acquisition, and monitoring progress.
- Link clients with community social service providers, healthcare providers, substance abuse, and mental health providers.
- Perform utilization review of patient cases, both inpatient and outpatient, and review medical necessity of referrals to specialty providers.
- Work with a multidisciplinary team to promote efficient and effective use of various healthcare resources.
- Focus on providing quality care, decreasing the fragmentation of care, enhancing patient self-care and quality of life, and cost containment.
- Collaborate closely with healthcare providers to ensure implementation of appropriate resources.
- Responsible to the Director of Case Management/Care Coordination.
- Must demonstrate knowledge of electronic health record systems.
- Participate in regular orientation and ongoing training sessions.
- Compliance with COVID vaccination requirements; no religious exemptions are accepted, and medical exemptions require approval.
