Job Title: Social Worker - MSW
Job Specialty: Home Health and Hospice
Job Duration: 13 weeks
Shift: 8 Hour Days
Guaranteed Hours: 40 hours per week
Experience: Minimum 1.5 years in Home Health/Hospice
License: Eligible for and willing to obtain CSW license; CSW or CSW-PIP required
Certifications: BLS through the American Heart Association
Must-Have: Master's Degree in Social Work
Job Description:
- Provide social work services to Hospice and Home Health patients within a home setting.
- Collaborate effectively with the Interdisciplinary Group (IDG) to meet Home Health and Hospice criteria and regulatory requirements.
- Independently manage schedules, typically visiting up to 5-6 patients per 8-hour day.
- Participate in a weekly case conference, the process, and schedule of which will be explained during the interview.
- Comply with documentation requirements using Epic and the Rover App.
- Required to have a valid driver's license and vehicle insurance.
- Reimbursement for mileage will follow the current IRS rate.
- Skills required include psychosocial support, patient/family needs assessment, resource finding, and bereavement support.
- Provide cross-coverage between multiple locations with an average drive time that varies, recommending housing in specific areas.
- Orientation includes minimally one week of shadowing/training and one day of online training.
- Dress code is casual.
- Manage visits autonomously, with leadership support available by phone.
- Participate in interdisciplinary team meetings with a Medical Director.
- Responsible for ensuring HIPAA compliance while using secure instant messaging for communication.
- No weekend rotation, call, or holiday expectations; however, visits may last longer in crisis situations.
