The Role:
The Clinical Social Worker will be responsible for elevating the level of care we provide to clients and their families. The Social Worker will provide support and advocacy to help address various social and emotional challenges that our clients and their families may face on their home care journey. The Social Worker will leverage their clinical expertise to provide this care in hospitals, senior living communities and in clients' homes. The Social Worker will apply independent judgment to guide and respond to medical professionals and clinical professionals in varied healthcare settings and should possess keen knowledge of the healthcare industry necessary to integrate within concierge medicine networks, specialist groups, care management firms, and acute and post-acute care settings through pre-planned encounters and meetings. You will be a trusted partner among these channels and their referrals to navigate immediate home care needs.

The net result of your efforts will allow clients to remain healthy in their homes with better outcomes and improved longevity by having access to premium care at home.
The territory for this role is NYC.
What You Will Do:

s a member of the Clinical team at Alvita Care, the Clinical Social Worker is expected to support Operations in the provision of home care for our clients.
  • Overall responsibilities of the Clinical Social Worker position include the assessment, planning development, coordination, and monitoring of care services to ensure the highest quality of care for ongoing client cases in the NYC market. Case Management - This role plays an integral part in coordinating care and providing clients with the necessary resources, options and support for a healthy care outcome; this role also acts as an advocate for patients and their families, linking them to other members of the care team and helping them navigate the complexities of the health care system.
  • Serves as the primary point of contact for families and clients for all non-clinical care matters; conducts and directs meetings and calls with clients and their families
  • Coordinates with the Client Care team to prepare caregivers and to ensure that the Plan of Care is being correctly executed
  • Participates in weekly Client Care Review meetings in the NYC care center with other members of the care team
  • Will provide an average of 20 hours a week of case management services as directed by the Director of Nursing
  • Establish, grow, and maintain relationships in assigned territory
  • Maintain communication with the Sales team to understand case nuances, changing client needs and requests for GCM support

  • Effectively manage all administrative duties of the role, including but not limited to client care visit documentation, advancement of the sale, expense reporting, etc.

Who You Will Be:
  • Social Worker with master's degree in social work from an accredited university; licensed in State of New York (CSW or LSW required)
  • 10+ years of case management experience is preferred
  • The ability to work in various clinical environments
  • Strong interpersonal communication, influencing, critical thinking, and problem-solving skills required
