Title: Licensed Practical Nurse (LPN) - Skilled Nursing Unit
Location: Coaldale, PA
Position Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Skilled Nursing Unit. The LPN will provide direct patient care in a skilled nursing environment, working alongside a team of healthcare professionals to ensure the highest standards of patient care. The ideal candidate will be responsible for assessing patient conditions, administering treatments, and contributing to the development of care plans.
Key Responsibilities
Provide nursing care to residents, including medication management, wound care, and other prescribed treatments.
Monitor vital signs, track patient progress, and report any changes to the registered nurse or physician.
Assist in developing and implementing individualized care plans to ensure personalized care.
Educate and provide emotional support to residents and their families regarding medical conditions, treatment plans, and recovery.
Assist residents with daily activities, including mobility, personal hygiene, and feeding while ensuring dignity and comfort.
Maintain accurate patient records in electronic medical systems.
Collaborate with nurses, physicians, therapists, and social workers to ensure comprehensive patient care.
Follow infection control and safety protocols to maintain a safe environment.
Respond to medical emergencies promptly and provide immediate care.
Participate in ongoing training and education to stay updated on best practices.
Qualifications
Current LPN license in the state of Pennsylvania.
Previous experience in skilled nursing, long-term care, or a similar healthcare setting preferred.
Strong interpersonal and communication skills, with the ability to work effectively as part of a team.
Compassionate and patient-focused approach to care.
Ability to handle medical situations professionally.
Working Environment
