Job Objective:
Under the direction/oversight of Eisenhower Concierge Services (ECS) executive leadership, provides direct patient care within scope of license, and collaborates with patients, families, peers, physicians and other members of the health care team to deliver exceptional nursing care services in the home or other site of service, per client/patient/licensed provider request.

Required: Graduate of an accredited Licensed Vocation Nurse Program

Licensure/Certification:
Required: California Licensed Vocational Nurse (LVN); Intravenous Therapy and Blood Withdrawal Certification; American Heart Association BLS; California Driver License and auto insurance

Experience:
Required: One (1) year of LVN experience in a direct patient care clinical setting
Preferred: LVN experience in a hospital

Essential Responsibilities
1. Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
2. Complies with all applicable ECS policies and procedures, including Code of Conduct and Compliance policies, and Relationship-Based Care Professional Practice Model; takes action to involve ECS leadership to resolve compliance questions or concerns and/or reports suspected violations.
3. Recognizes potentially critical situations and takes action to minimize adverse outcomes.
4. Develops and maintains open, honest and mutually beneficial relationships with ECS leadership, staff, clients, and relevant stakeholders.
5. Collaborates with ECS leadership/peers to continuously improve/streamline workflow processes to ensure and uphold high-caliber service and conserve valuable resources.
6. Follows policies, procedures, directives and works with executive leadership to update/improve when/where appropriate.
7. Collaborates tightly ECS leadership and Care Coordinators to appropriately coordinate caregiver schedules for the provision of client care.
8. Facilitates client's participation in all decisions concerning care and appropriately advocates on their behalf.
9. Direct patient care, as prescribed by ECS policy and procedure.
10. Implements plan of care according to identified physical, psychosocial, and spiritual needs of patient.
11. Assists other staff in the delivery of care.
12. Ensures physician orders to initiate treatments and scheduling are available and viable; judiciously intervenes/connects with physicians to rectify issues.
13. Ensures materials/goods utilized within the course of nursing care are captured for appropriate billing to clients.
14. Appropriately handles sensitive electronic hardware (cell phones, laptops, clinical equipment) so they remain in good working order, are not left unattended, and makes sure to follow check-out/in procedure so they remain available and operational for other staff.
15. Recognizes, accepts responsibility for, and maintains commitment to professional development and clinical competency.
16. Shares knowledge and acts as a professional resource to other members of the health care team, especially in relation to new hire onboarding.
17. Embraces change and continuously demonstrates a commitment to utilize evidence-based practice.
18. Actively seeks opportunities to support/up-lift ECS morale.
19. Earnestly promotes a positive organization image when interfacing with the public/clients, medical system staff/ professionals, and either personally takes action to address issues and/or immediately informs and engages ECS executive leadership.
20. Takes call for ECS on a rotational basis, and ensures personal work availability is adequately flexible to meet operational need.
21. Maintains reliable transportation and must be willing to utilize personal vehicle to transport oneself to site of contracted client. Engage in honest, accurate accounting of miles traveled. (Mileage reimbursed at current IRS defined rate per mile).
22. Performs other duties as assigned.
