Porters Neck Village is looking for an exceptional Health and Wellness Navigator who wants to work in an independent living retirement community with a country club feel. Position is Monday-Friday 8am-4:30pm. Nights and weekends may be required at times based on case load/needs. Excellent work environment! Let us schedule an interview with you today!

The Health & Wellness Navigator is responsible for building relationships with residents and families, gaining trust, solving problems, being a resource, supporting, and assisting residents to maximize their highest level of health and well-being, helping residents access available services and resources within and outside the community to reduce barriers for successful health and wellness to maintain independent living.

The Health & Wellness Navigator assists residents in facilitating their health and wellness goals within the framework of the 8-Dimensions of Health & Wellness model including, but not limited to intellectual, occupational, social, environmental, spiritual, emotional, physical, and health services, personal choice, and provide as much support as residents need to maintain overall well-being throughout their journey within the community.

The Health & Wellness Navigator, when necessary, will assist and guide residents, family members, and or caregivers with resident transitions throughout the continuum of care to achieve the optimal level of well-being and appropriate level of care and communicate with all key resources and stakeholders.

Here are a few of the qualifications we need you to have:

  • Three to five years of experience working in the senior living space or hospital base setting with strong organization skills, management experience with care coordination and care management with an interdisciplinary approach to care preferred.
  • Graduate of an accredited college with an RN, or bachelor's degree in social services, geriatrics, nursing, and/or related senior living services preferred.
  • Valid driver license with satisfactory driving record
  • Willingness to be available for any/all emergencies regarding the community

Pay: $85,000/year with bonus potential.
