Job Title: Social Worker - MSW
Job Specialty: Home Health
Job Duration: 13 Weeks
Shift: 8-Hour Days
Guaranteed Hours: 40 Hours/Week
Experience: Minimum of 1.5 years, including Home Health/Hospice
License: SD Licensure for Social Work required; must be eligible and willing to obtain CSW license
Certifications: BLS through AHA
Must-Have: MSW - Master's Degree in Social Work; Valid Driver's License; Vehicle Insurance
Job Description:
- Provide psychosocial support and hospice skills, including assessment of patient and family needs, resource finding, and bereavement support.
- Collaborate with the Interdisciplinary Group through the IDG process to meet both home health and hospice criteria and regulatory requirements.
- Field position requiring commuting and providing care within the home setting.
- Participate in weekly case conferences to discuss patients with identified needs.
- Autonomy in managing daily schedules, typically visiting up to 5-6 patients per day.
- Possess skills in using Epic Remote Client and Rover App for electronic verification of visit times and HIPAA compliant communication.
- Cross-coverage between specified locations is required, with recommended housing in certain areas.
- Orientation includes minimally one week of shadowing and training with other caregivers and one day of online training.
- Casual attire is expected.
- Leadership is always available by phone for support.
- No on-call requirements, but visits may extend if a patient is in crisis within the Hospice environment.
- Approval of time off needed through the contract.
