Job Title: Registered Nurse

Job Specialty: Patient Care Coordinator - Critical Need

Job Duration: 13 weeks

Shift: 8 hours, 08:00 AM to 04:30 PM

Guaranteed Hours: 40 per week

Experience: Minimum of 1 year in case management or as a Patient Care Coordinator

License: Active RN license or compact license

Certifications: BLS

Must-Have:
- Experience with EPIC charting
- Familiarity with Medicaid/Medicare
- Availability to work weekends and holidays

Job Description:
- Coordinate patient care and ensure critical needs are met efficiently.
- Manage patient cases with precision, offering support where needed.
- Utilize EPIC charting system to maintain accurate patient records.
- Navigate Medicaid/Medicare processes effectively.
- Work collaboratively with healthcare team to enhance patient outcomes.
