We are onboarding new employees using a hybrid approach. The Department supports flexible work arrangements. Depending on the business need and description of the position, we have options that range from hybrid, to full-time in the office. The Compliance and Innovation Division is responsible for ensuring the Office of Community Living can be responsive to the Department of Justice (DOJ) findings, the Olmstead decision, and the Americans with Disabilities Act. It will be responsible for transitioning individuals out of more restrictive or institutional settings.

The Access Unit exists to support individuals in need of access to Long Term Services and Supports. The unit oversees Transition Services, Preadmission Screening and Residential Review (PASRR), LTSS Presumptive Eligibility, Money Follows the Person (MFP), housing support, and other OCL program functions, as necessary. This unit works in coordination with other divisions to align with parallel and ongoing efforts to ensure that programming is consistent with all state and federal policy guidance. It is the goal of the Access Unit to support Medicaid organizations that help individuals access adequate care; regardless of age, disability or geographic region. The Unit plays an important role in working toward the Department's mission to improve access to cost-effective, quality long term care services and support for Coloradoans.
Description of Job

What you'll be doing:

The Transitions Administrator oversees the Department's Transition Coordination program and is responsible for the ongoing performance and quality of the Transition Coordination Agencies (TCA) who provide the service. Position is the Staff Authority for the Transition Coordination program and oversees the Targeted Case Management-Transition Coordination (TCM-TC) benefit. This position works with the TCAs to support program quality and critical member issues. This position is responsible for program training and communications and works across teams to ensure that information is accurate and accessible. The Transitions Administrator provides subject matter expertise to related program development and implementation efforts. Position provides regulatory oversight to the program and guides stakeholder engagement efforts as appropriate.

MINIMUM QUALIFICATIONS:
There are two ways to qualify for this position:
Option 1:

Experience Only:

Seven (7) years of related professional experience in project management, program development, program coordination and/or implementation.
Option 2:

Education and Experience:

A bachelors degree from an accredited institution in health care administration/management, human services, public health care policy, or in a field of study related to the work assignment AND three years of professional experience in project management, program development, program coordination and/or implementation
Appropriate education will substitute for the required experience on a year-for-year basis. Relevant education will be calculated as the following:
  • An associate will count as two years of experience
  • A bachelors will count as four years of experience
  • A masters degree will count for six years of experience
  • A doctorate or juris doctorate degree will count for seven years of experience


PREFERRED QUALIFICATIONS
  • Prior state experience related to the duties of this position
  • Experience related to public policy development.
  • Experience in piloting a program
  • Prior experience working in government
  • Experience researching and interpreting complex state and federal rules and regulations

DEFINITION OF PROFESSIONAL EXPERIENCE: Work that involves exercising discretion, analytical skill, judgment, personal accountability, and responsibility for creating, developing, integrating, applying, and sharing an organized body of knowledge that characteristically is uniquely acquired through an intense education or training regimen at a recognized college or university; equivalent to the curriculum requirements for a bachelor's or higher degree with major study in or pertinent to the specialized field; and continuously studied to explore, extend, and use additional discoveries, interpretations, and application and to improve data, materials, equipment, applications and methods.
CONDITIONS OF EMPLOYMENT:
  • All positions at HCPF are security sensitive positions and require that the individuals undergo a criminal record background check as a condition of employment.
