Job Description
Job Title: RN - Patient Care Coordinator

Job Specialty: Critical Need - Outpatient Staff Lead

Job Duration: 13 weeks

Shift: 8 hours, 08:00 AM to 04:30 PM

Guaranteed Hours: 40 hours per week

Experience: Minimum of 1 year in case management or as a Patient Care Coordinator

License: Active RN license

Certifications: BLS

Must-Have:
- Experience with EPIC charting
- Familiarity with Medicaid/Medicare
- Available to work on weekends, including holidays
- Preference for candidates with previous experience in similar roles

Job Description:

- Coordinate patient care efficiently and effectively
- Ensure high standards of clinical practice and maintain regulatory compliance
- Collaborate with interdisciplinary teams to develop individualized patient care plans
- Provide education and support to patients and their families
- Utilize EPIC for documentation and maintaining patient records
- Demonstrate excellent communication skills and patient advocacy
- Adapt to varying patient needs and manage multiple tasks efficiently

This role is critical in providing exceptional outpatient care and requires a candidate with strong organizational skills and a compassionate approach.
