Job Title: Licensed Practical Nurse (LPN) - Full Time Float
Job Specialty: Long-Term Care (LTC) LPN
Job Duration: 13 weeks
Shift: Full-Time Day/Evening Rotation (5x8 per week, potential for 3x12s if availability permits), Every Other Weekend, Holidays Required
Guaranteed Hours: Not specified
Experience: 1 year of experience as an LPN, preferably in an LTC Facility
License: Valid LPN Licensure
Certifications: Basic Life Support (BLS) certification through the American Heart Association (AHA) or RQI
Must-Have: Ability to provide care to adult and geriatric patients
Job Description:
- Demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served.
- Understand the principles of growth and development across the lifespan.
- Assess and interpret data reflective of the patient's status to identify age-specific needs.
- Provide both direct and indirect skilled nursing care for assigned residents.
- Follow defined policies and procedures under the supervision of a Registered Nurse.
- Maintain collaborative relationships with nursing staff, physicians, and ancillary staff.
- Maintain knowledge and clinical expertise through continuing education and in-service programs.
- Collect data on residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
- Contribute to creating a developmentally age-appropriate and culturally relevant plan of care reflecting nursing diagnosis and interventions.
- Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals as needed.
- Collaborate with the resident, family, and interdisciplinary team, ensuring customer satisfaction.
- Maintain current knowledge and competency in nursing practice and participate in staff mentoring and orientation.
- Consider factors related to safety, cost, and impact in planning and delivering care.
- Provide leadership in the professional practice setting under the direction of the RN.
- Anticipate occupational exposure to bloodborne pathogens and manage tasks involving potential contact with blood, body fluids, or tissues responsibly.
