Pay Rate: $48 - $52 per hour
Shift: Monday - Friday, 8:30 AM - 4:30 PM (Includes 30-minute break)

Job Summary:

We are seeking a Licensed Master Social Worker (LMSW) to join our Care Coordination & Social Work Department. The ideal candidate will provide comprehensive social work services, support care coordination, and assist patients and families in navigating healthcare resources.
Responsibilities:

• Conduct patient interviews, assessments, and develop care plans.
• Provide psychosocial support and crisis intervention.
• Assist with case management, discharge planning, and referrals.
• Coordinate transportation and community resources using Allscripts referral management.
• Work with psychiatric and pediatric populations as needed.
• Collaborate with multidisciplinary teams to enhance patient care.
• Maintain accurate documentation in EPIC EMR.

Qualifications:
Required:

• Master's Degree in Social Work (MSW)
• New York State LMSW License
• Strong interviewing, judgment, and decision-making skills

Preferred:

• Previous hospital social work experience
• Case management experience
• Bilingual in Chinese or Spanish
