Salary: $90,000.00 - $100,000 per year + benefits

About This Opportunity:

Our team is currently looking for a licensed Clinical Social Worker who will work collaboratively with the interdisciplinary treatment team as a patient advocate. The team's approach is to work with our patient population to assist in chronic disease management through therapeutic interventions, clinic support, and ongoing education.

This is a full-time, 40-hour-per-week, benefit-eligible, fully remote opportunity with a Monday - Friday schedule. No weekends or Holidays!

Job Duties:
  • Complete initial assessment of the identified patient, assessing client's strengths, weakness, challenges, and abilities, and ensure all appropriate intake information and documentation is completed
  • Utilizes specific therapeutic modalities that will best suit client needs.
  • Identify specific psychological interventions to assist client in reaching their goals
  • Provide individual and/or family counseling
  • Provide vocational, experiential, and family counseling/coaching as needed
  • Collaborate with other community resources and referral sources to coordinate services for client
  • Provide crisis intervention and documentation when necessary
  • The clinician will provide over-the-phone assistance to the family and/or client when necessary
  • Maintains appropriate state license for scope of practice
  • Provide feedback and consultation to treatment team members regarding patient presentation and progress


Requirements
  • Master's Degree in Social Work from an accredited graduate program
  • 1 Year Experience in a behavioral health setting
  • California BBS Licensed Clinical Social Worker (LCSW) - CA Board of Behavioral Sciences
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association
  • Excellent organizational and critical-thinking skills
  • Knowledge of and ability to apply practical assessment, interviewing, and counseling techniques
