MAJOR RESPONSIBILITIES/ESSENTIAL FUNCTIONS
The Rehab Clinical Liaison ls a highly visible, challenging and rewarding position. The liaison works to build and maintain inpatient census for the acute rehabilitation unit by conducting thorough clinical evaluations of patient referrals, facilitating admissions through collaboration with other rehabilitation disciplines and physicians. They will assume a leadership role as the primary contact for referrals; developing and nurturing professional relationships with case managers, physicians and clinicians in acute care hospitals and educating staff, patients and families regarding rehabilitation services provided by unit. They will assess patients with rehab diagnoses and coordinate the referral process and admission conversion process.

- Initiates the pre-admission screening process upon receiving patient referrals, complete thorough clinical evaluations, and facilitates prompt admission decisions as guided by insurance data, established government criteria, medical therapy, and patient's social support
- Creates target lists of contacts, develops relationships with referral sources, providing excellent customer service and education about the HPMC Rehabilitation Program
- Recognizes barriers to admission, responds appropriately and follows up on admission
- Understands healthcare operations, legal guidelines, competitive analysis, and market place trends
- Provides in-services and professional presentations for referral sources regarding HPMC rehabilitation program and outcomes
JOB QUALIFICATIONS
Minimum Education
- Bachelor's degree
- Registered Nurse License
- Graduate of an RN training program or Allied Health Professional (PT, OT, Speech) Program
Preferred Education
- Registered Nurse License, Physical Therapist or Occupational Therapist or Speech Language Pathologist Therapy license
Minimum Work Experience and Qualifications
- One year related experience working in a rehabilitation setting or in another post-acute care setting.
Preferred Work Experience and Qualifications
- Two years related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role preferred.
- Demonstrated competency with the preadmission process including assessments and relationship building in a rehabilitation setting or in another post-acute care setting.
- Demonstrated computer proficiency
Required Licensure, Certification, Registration or Designation
- Current BLS Certification
- Assault Response Competency (ARC) required within 30 days of hire
- Fire Card required within 30 days of hire
