Job Title: Registered Nurse (RN) Case Manager
Job Specialty: Case Management/Discharge Planning
Job Duration: 13 weeks
Shift: 8-hour days, Monday-Friday
Guaranteed Hours: 40 hours per week
Experience: Minimum of 2 years
License: RN License required
Certifications: Basic Life Support (BLS), Case Manager Certification (CCM) preferred but not required
Must-Have:
- At least 2 years of acute care management experience in discharge planning.
- 6 months experience in a level 1 or large academic medical center hospital.
- Proficiency in EPIC charting.
Job Description:
- Perform case management and discharge planning for inpatient units across the hospital.
- Collaborate with a team of 15 full-time and 17 part-time social workers.
- Handle a caseload of 15-20 patients/consults per day.
- Consult with respiratory therapy and pharmacy for discharge planning as needed.
- Participate in department-specific orientation for 2 weeks.
- Report directly to the Social Work Manager, with oversight provided by the Chief of Care Coordination.
- No weekend requirements; holidays are rotated as needed.
- Maintain a professional appearance, wearing any color scrubs or business professional attire.
- Join a growing team aiming to expand from 15 to 21 case managers.
