Position Summary:

The Associate Executive Director of Continuous Qualit(AED of CQI)is responsible for creating and implementing a Continuous Quality Improvement program to establish a culture of ongoing improvement, focusing on identifying and addressing issues, implementing solutions, and consistently monitoring and refining processes to achieve better outcomes of our services.

The AED of CQI will work collaboratively with program leaders to implement and support CQI practices that align with agency strategy and regulatory requirements unique to that program.

Acting as the agency's Chief Compliance Officer, the AED of QCI is responsible for overseeing compliance with state laws and regulatory requirements, accreditation bodies, and SCCC policies and procedures.

Position Requirements & Specifications:
  • Ability to work: Monday - Friday, 8:30am - 4:30pm w/ flexibility as needed.
  • Bachelor's degree required; Master's degree preferred.
  • Four+ years of QA experience may be substituted for a portion of the educational requirements.
  • Strong organization and planning skills with the ability to effectively communicate them to all levels of staff.
  • Possesses extensive knowledge of guidelines and regulations mandated by accreditation and regulatory bodies including but not limited to: Office of Children and Family Services (OCFS), Office of Mental Health (OMH), State Education Department (SED), Justice Center (JC) and St. Catherine's Center for Children's policies and procedures.
  • Certain deadlines and unanticipated developments may require flexibility to work during evening and/or weekends hours.
  • Proficiency in various computer applications such as Microsoft Office Suite, Google Business Workspace Suite, and Awards; and use secure Internet practices.
  • Clean and Valid New York State Driver's License (actively licensed at least one year) is required. Staff in driving positions must be able to meet the minimum requirements of our agency's insurance policy for authorized drivers; including, but not limited to the minimum age of 21 at time of hire.
  • Have a professional record of integrity, proven reliability and dependability; with qualities of positivity, flexibility and adaptability.
  • Capable of exercising ethical, independent judgment and discretion, and handle sensitive data with strict confidentiality.
  • Ability to pass pre-employment background check requirements, including fingerprinting.

Essential Duties & Responsibilities include:
  • Direct and supervise the Quality Assurance Specialists.
  • Ensuring the development and ongoing updates, as state regulations, accreditation guidelines and agency policy and procedure change, of QA checklists for compliance walk-throughs and record reviews.
  • Ensuring compliance to regulatory requirements, accreditation guidelines and agency policy and procedure.
  • As a member of the Board Policy Committee, ensures Agency Policy and Procedures are up to date, reviewed regularly and current.
  • Work closely with program leadership to provide QA assistance within their programs for specific time periods, special projects, monitor specific staff or identified problematic areas, etc.
  • Ensure the Staff Development & Training Department is working closely with program leaders to monitor training needs to ensure staff can meet the requirements of their specific job responsibilities and work in alignment with St. Catherine's values and service philosophy.
  • Identify ongoing staff development needs through regular announced and unannounced program visits.
  • Provide in person new hire orientation in conjunction with Human Resources covering Medicaid compliance, HIPAA, Agency Silent Whistle, Privacy Practices and various other compliance related policies and procedures.
  • Serves as the agency HIPAA Compliance officer; Privacy Officer.
  • Maintain the Agency's Silent Whistle Platform and the primary point of contact. Investigate any allegations of alleged non-compliance, tracking all issues and making recommendations to mitigate identified areas of concern and/or risk to agency clients, staff or the agency as a whole.
  • Maintain updated knowledge and expertise in the use of the Agency's Electronic Health Record to ensure compliance to state regulations pertaining to client records; HIPAA; PHI and Medicaid in close collaboration with the Electronic Health Records Specialist.
  • Maintain a working knowledge and ability to effectively use other electronic record keeping programs such as, but not limited to, Connections, IEP Direct, Binti, Extended Reach.
  • Remain current on all Medicaid regulations, Office of Children and Family Services (OCFS), State Education Department (SED), Office of Mental Health (OMH),Department of Health (DOH), and other oversight agencies
  • Working closely with and as the primary contact for JC, OCFS, DOH, SED, OMH and other oversight agencies, properly address any significant incident reports or investigations.
  • Track and monitor trends and data as it pertains to significant incidents and investigations. Make recommendations to mitigate identified areas of concern and/or risk to agency clients, staff or the agency as a whole.
  • Ensure Performance Improvement Plans issued by a regulatory body are completed in a timely and thorough manner, and communicated to the Executive Director when either verbal or written notice is given.
  • Work with Programs to ensure data driven metrics are being utilized as a tool to assist with decision making regarding care, treatment, and services to clients.
  • Participate as the lead of the Continuous Quality Improvement Board of Directors Committee, preparing and presenting clear and concise reports.
  • Lead the CQI Committee that reviews all trends, incidents, and PIP's.
  • Complete an annual Risk Assessment.
  • Complete annual staff satisfaction survey in conjunction with HR.
  • Sit on the Board Policy Committee, which reviews and updates all Agency policies.
  • Develop the annual compliance work-plan based upon an annual Risk Assessment; revising throughout the year as needed.
  • Assist with planning, coordination, and implementation of new projects and new service locations as directed by the Executive Director.
  • Maintain all institutional reports and communications.
  • Participate in all tours for annual inspections required by regulatory bodies as scheduled, and follow up with documented corrective actions taken.
  • Act as primary point of contact for the Council of Accreditation (COA):
  • Additional position-related duties and responsibilities as assigned by the Executive Director.
