Hours: Full Time Monday-Friday, 8:00AM-4:30PM Flexibility to work evenings and weekends may be required based on need.

GENERAL STATEMENT OF RESPONSIBILITIES:

Under direct supervision of the Chief Medical Officer of Quality Care (CMOQC), the Registered Dietitian/Healthy Lifestyle Coach (RD/HLC) will work closely with the Chief Medical Officer of Quality Care (CMOQC) and others as identified to provide dietitian services and participate in Healthy Lifestyle/Wellness coaching. Dietitian services will be available with primary focus on patients with diabetes mellitus, but also when indicated and referred by physician for dietitian services related to obesity, childhood nutrition, and for medically indicated condition specific diets. The RD/HLC will coordinate with the Medical Director and the clinical staff to provide opportunities to discuss and guide patients/clients with diabetes related meal planning and lifestyle choices, both in community based and one-on-one consultation. The RD/HLC will work to ensure that senior meals meet dietary guidelines per the USDA as well. This position will actively work for the improvement of community services available and continued participation of community members who attend educational opportunities. The RD/HLC will work as part of a team that supports the efforts of the physician, nurse, community health point of contact, and consultants related to this project/program. The coordination of these services will allow for a comprehensive community health nutrition, exercise, and wellness program.

SPECIFIC DUTIES AND RESPONSIBILITIES:

  • Host community engagement classes biweekly that incorporate topics of healthy lifestyles including but not limited to: physical activity, social environment, and cooking/meal planning in a culturally sensitive context.
  • Provides diabetic specific nutrition education at these classes.
  • Develops a program with measurable results identified for review.


  • Meets one-on-one with individuals as requested and referred by the physician.


  • Creates and actively follows up through evaluation of created plan.


  • Develop a tool to address the effectiveness of the plan and revises as appropriate.
  • Provides opportunities to develop a plan including implementation process to include, but not limited to: necessary lifestyle changes for individuals and families.
  • Maintains necessary records to establish priority analysis and evaluation for care.


  • Collaborates with supervisory physician in the development of case management of diabetic patients and families.


  • Conducts comprehensive and focused assessments including functional assessments of diabetic populations. Identifies health problems by observation, interviews, and analysis of records.


  • Provides on-going educational reinforcement as requested by medical staff. Establishes mutually agreed-upon plans of care with patients.


  • Consults with integrated healthcare team which includes, but is not limited to: physician, nurse, and mental health. Appropriately refers patients based on plan of care.


  • Implements health teaching with an emphasis on disease prevention and health promotion to individuals and groups.


  • Develops and secures culturally appropriate informational and educational materials, guides, brochures, etc.


  • Collaborates with physicians and clinical staff to assess, plan and implement strategies to advance clinical practice and educational needs of patients and staff.


  • Attends staff meetings, in-services and appropriate courses to maintain personal and professional growth.


  • Participates in community activities, committees, workshops and seminars as assigned by the SDPI Coordinator.


  • Works with the Title VI Staff to review the monthly senior meal menu.


  • Maintains certification in field of specialty area once obtained.


  • Maintains current practice guidelines for the clinic.


  • Fosters team work; demonstrates ability to communicate with and work with staff and other departments.


  • Maintains patient confidentiality.


  • Performs other duties as assigned.


  • QUALIFICATIONS:

    Education/Experience: A bachelor's degree and at least 2-4 years of experience in the field or in a related area is required. Must have a valid Registered Dietitian License. A Certified Diabetes Educator (CDE) is preferred. Must be 18 years of age or older.

    Licenses/Certifications: A valid California driver's license required with application submission and must be maintained throughout employment.

    Applicant must be insurable under SIHC vehicle insurance policy at the time of hire and throughout employment.

    Current certifications and/or licenses appropriate to the positions required education and profession.

    Character: Applicants must have a reputation for honesty and trustworthiness. Must be responsible and able to exercise good judgment, accept administrative supervision, pay attention to detail, follow instructions, including the ability to interact effectively and communicate with people in a professional and courteous manner. Must be highly confidential and work as a team with other staff. Applicant should be sensitive to client's needs.

    Skills: Strong team building, interpersonal, and organizational skills. Ability to establish and maintain effective peer relationships with coworkers within the Department, clinic-wide, and the public. Must be able to express ideas clearly, concisely, address audiences effectively, and exercise balanced judgment in evaluating situations and making decisions. Willing to be part of a team and cooperate in accomplishing department goals and objectives. Ability to prioritize, meet deadlines, take initiative, be proactive, and function in a rapidly changing environment. Ability to work with people of all social and ethnic backgrounds and to resolve conflicts, negotiates situations, and facilitate consensus.

    Physical and Personal Requirements: Normal clinic/office environment. Sit or stand for long periods of time. Reach, bend, climb, stoop, lift up to 25 lbs. repetitive hand movement; use and view PC. Reliable transportation and car insurance as required by the state. Travel as needed.

    Compliance: Each employee plays a key role in creating a compliance culture at SIHC. Employees are expected to learn and comply with all SIHC policies and procedures. One must have the ability to understand the implications and complexities of all compliance policies. Participation in all compliance training is mandatory, and generally have a heightened awareness of policies and regulations that are specific to one's department and position. All SIHC employees are expected to report violations or suspected violations of compliance policies or regulations.

    Other: Applicants must successfully pass a pre-screening tuberculin skin test or x-ray and a blood/urine drug screening test. Health must be adequate to perform all duties of the position. Applicant must complete SIHC Application and Authorization Form, both must be submitted to Human Resources prior to the close date indicated.

    INDIAN PREFERENCE:

    INDIAN PREFERENCE: In the filling of any SIHC job vacancy, preference may be given to qualified Native American Indians, pursuant to the Indian Preference Hiring Act, 25 USC 472, unless other laws require the filling of a vacancy without regard to Indian preference (e.g. Public Law (P.L.) 94-437, "Indian Health Care Improvement Act (IHCIA). To receive Indian preference for any SIHC position, the applicant must be enrolled, or be eligible for enrollment, as an American Indian with their Tribe, or must be certified as an American Indian from the designated Tribal Representative. Applicants claiming Indian preference must attach verification of their claim to the SIHC application, including Certification of form BIA-4432, which is available from the SIHC Human Resources Department. If verification is not or cannot be verified, the applicant will not receive Indian preference for purposes of the interview.
