Division and Program Description
Felton's Senior Division specializes in a full continuum of mental health services for older adults in outpatient clinics, the community, and supportive housing sites, as well as programs in employment training, economic security, case management, advocacy, and social connection and inclusion.
Felton's Clinical Housing Support Program provides behavioral health and other onsite support services to assist tenants at the Rene Cazenave Apartments (RCA) to maintain housing stability and improve access to resources. All tenants were formerly homeless adults with co-occurring mental health, substance abuse, and medical problems, and limited experience living independently. Support services include individual and group behavioral health counseling and case management, psychiatry, primary care nursing case management and medication monitoring, referral to and coordination with primary medical care, substance abuse and psychiatric treatment, benefit counseling and client advocacy, meal programs, health education, community building, tenant organizing, and all other case management and support functions.
Job Description
Under the supervision of the Program Supervisor, this position participates in a multidisciplinary team serving the tenants with comprehensive and integrated treatment services. The RN provides primary care nursing case management, medication dispensing and monitoring, and referral to and coordination with primary medical care. The Clinical Housing Support RN evaluates needs of clients and develops treatment plans, with the clinical case manager and the rest of the resident's treatment team.

Salary Range: $140,000-$168,000 Annually (DOE)

Job Description

The Clinical Housing Support RN evaluates needs of clients and develops treatment plans, with the clinical case manager and the rest of the resident's treatment team.
Specific Responsibilities
- Works closely with an interdisciplinary team of clinical case managers, psychiatric nurse practitioners, psychiatrists, and community specialists (peer counselors) to provide onsite support to residents in the RCA apartments.
- Works closely with inter-agency treatment teams for each resident receiving medication support services to coordinate medical and psychiatric treatment, including appointments and medications.
- Coordinates with pharmacy to ensure smooth delivery and administration of medications.
- Creates a welcoming community milieu within the apartment building by supporting community events.
- Collaborates with property management to support tenants to maintain stable independent housing.
- Participates in Officer of the Day rotation to be point person to manage crises, drop ins and wellness checks.
- Provides crisis intervention and management to prevent eviction and hospitalization, and if necessary, facilitates involuntary hospitalization.
- Provides health education to improve wellness and prevent further medical issues.
- Maintains thorough clinical records after each client contact or service, in accordance with mandated standards of San Francisco Community
- Mental Health Services and funders including Medi-Cal/Medicare.
- Participates in team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
- Ongoing training by participating in Felton's training opportunities, including developing specializations in evidence-informed treatments and practices.
- Other duties and responsibilities that may be assigned from time to time.
Minimum Qualifications
- Valid Registered Nurse (RN) license in California and current CPR certification.
- Minimum of 2 years of nursing experience, preferably in a healthcare setting serving vulnerable populations in behavioral health.
- Strong clinical assessment and critical thinking skills, with the ability to make sound judgments and decisions under pressure.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust with residents, families, and team members.
- Compassionate and empathetic attitude towards residents' physical, emotional, and social needs.
- Ability to work collaboratively in a multidisciplinary team environment, demonstrating flexibility and adaptability.
- Understanding of health care services within the Medi-Cal network in San Francisco
- Solution focused care planning and conceptualization skills within multi-disciplinary team and with external agency partners and individuals
- Ability to work well under pressure while meeting multiple different types of deadlines.
- Computer skills proficiency in applications for electronic health care records, Teams, Box, Microsoft/Outlook applications, and other technologies as needed for the role.
- Ability to be a team player with a welcoming attitude.
