Experience: Two to three years of recent clinical acute care experience required. Previous Case Management experience preferred.

License: Current licensure required

Certifications: Case Management Certification preferred

Must-Have: BSN required, MSN preferred

Job Description:

- The RN Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of the patient's plan of care from admission to post-discharge.
- Utilize clinical knowledge, critical thinking skills, and the principles of case management to coordinate and implement a discharge plan that meets the patient's needs and ensures a seamless, effective, and efficient transition of care across the continuum.
- Coordinate, negotiate, manage, and implement the care of complex patients to facilitate quality and cost-effective patient outcomes.
- Work collaboratively with interdisciplinary staff internal and external to the organization.
- Participate in quality improvement and evaluation processes related to the management of care.
- Responsible for overall delivery and quality of patient care in designated area(s) of responsibility.
- Assist with developing and implementing performance initiatives, ensuring optimal patient safety outcomes, and customer satisfaction.
- Manage daily work activities, ensuring quality, productivity, and efficiency while assisting in accomplishing strategic and operational objectives.

Candidates must possess effective communication and interpersonal skills, the ability to maintain confidentiality, adapt to changing priorities, and provide care that respects and responds to patient values in clinical decision-making.
