Job Title: Physical Therapist
Job Specialty: Home Health
Job Duration: 13 Weeks
Shift: Day, 5x8-Hour (08:00 - 17:00), Monday-Friday
Guaranteed Hours: Not Guaranteed
Experience: Minimum 1 full year in Adult Home Health
License: Must have a valid license
Certifications: CPR, BLS
Must-Have: Reliable transportation, current driver's license, and agency-required liability insurance
Job Description:
- Conduct home visits and attend periodic office meetings.
- Manage an average of 5-6 patients per day, translating to approximately 6-8 points.
- Responsibilities include start-of-care assessments and follow-ups.
- Required to travel up to 75 miles from the home clinic.
- Mileage reimbursed according to the IRS standard rate from patient home to patient home; commute to the first and from the last patient is not billable.
- Use EMR systems: HCHB and PointCare.
- Must be proficient in HomeCare HomeBase EMR and have used it within the last six months, or must complete a retraining course.
- All documentation must be completed within 24 hours of the appointment with the patient.
- Must hit weekly productivity expectations of 30 points.
- Opportunity to float to another nearby clinic to pick up patients if not receiving enough to support financial needs.
- Weekend rotation required every 4 weeks.
- No on-call responsibilities.
- Local candidates accepted at the same rate.
*Please ensure candidate understands they will drive to patient homes daily, and possibly commute longer distances between appointments. Candidates should be prepared to accept every patient scheduled regardless of distance, and delays in documentation can lead to contract cancellation. No guaranteed hours; productivity equates to approximately 40 hours per week.*
