Job Title: Registered Nurse

Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience
License: Must have license in hand
Certifications: CPR, BLS
Must-Have:
- Proficiency in HomeCare HomeBase (HCHB) and OASIS
- Reliable transportation, driver's license, and auto insurance
- Willingness to travel up to 75 miles from the home clinic
- Completion of all documentation within 24 hours of the patient appointment
Job Description:
- Coordinate and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process.
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Perform home visits and attend periodic office meetings.
- Manage a daily caseload of 5-10 patients, translating to roughly 6-8 points, with an expectation of 20-25 weekly units of productivity.
- Handle various types of cases, including Start of Care, Wound Care, and Follow-up Visits.
- Be open to occasional on-call shifts, approximately 1-2 times per month.
- Use Pointcare/HCHB EMR systems efficiently.

- Understanding that this is a home health assignment, involving daily travel to patients' homes and weekly clinic meetings.
- Comfort with potentially long drive times between patient visits.
- Willingness to accept every patient on the schedule regardless of travel distance.
