Job Description
Job Title: Physical Therapist (PT) Home Health
Job Specialty: Therapy/Rehabilitation
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: Not guaranteed
Experience: Minimum 1 full year of Adult Home Health experience
License: Must have current professional license in hand
Certifications: CPR, BLS
Must-Have: Reliable transportation, valid driver's license, and agency-required liability insurance
Job Description:
- Conduct home visits for therapy sessions with periodic office meetings.
- Manage an average of 5-10 patients per day, equivalent to approximately 6-8 points based on visit and activity type.
- Expected productivity is 28-30 points per week.
- Responsible for documentation and travel, which have different point values assigned.
- Must be knowledgeable in HomeCare HomeBase EMR and have experience with it within the last 6 months.
- Not guaranteed specific hours, but must achieve weekly productivity points equating to a 40-hour workweek.
- May require floating to nearby clinics if patient census is low to meet financial needs.
- Coverage may require travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS Standard rate from one patient's home to another.
- Perform types of cases including Start of Care and Follow-Up visits.
- No weekend rotation or on-call duties unless discussed with the hiring manager during the interview.
- Consultant is compensated for travel between patient homes, not for the commute to the first patient or return from the last patient.
- Documentation for each appointment must be completed within 24 hours to prevent impacts on billing and follow-up care.
- Open to both local and traveling clinicians, with locals commuting from their residence.
This is a home health assignment where clinicians will drive to patient homes daily and attend weekly meetings in the clinic. Understanding and acceptance of this responsibility are required.
